Aledade's 2024 Public Benefit Report Demonstrates Mission-Driven Health Care Impact

Report showcases Aledade’s progress in fulfilling its public benefit mission to deliver better health, better care and lower costs

Aledade, the nation’s largest network of independent primary care, today released its second annual Public Benefit Report, Doing More Good Together, that demonstrates how its purpose-driven model delivered record savings this year while advancing its mission of creating a health care system that is good for patients, practices and society.

As a Public Benefit Corporation, Aledade’s Board of Directors is driven to consider the interests of all stakeholders alongside its core public benefit purpose. This year's report illustrates the collective efforts of Aledade’s partners and stakeholders to transform the U.S. health care system through value-based care. The report includes an enterprise-wide analysis of the company’s actions, progress and accomplishments in 2024.

Highlights from the 2024 report include:

Doing Good for Patients: Aledade enables primary care organizations to deliver high-quality proactive, preventive care. In 2024, Aledade partners prevented 263,000 unnecessary hospitalizations and emergency department visits, conducted more than 800,000 annual wellness visits and achieved an 82% patient hypertension control score — 4% higher than the national average.

Doing Good for Practices: Aledade’s technology and expert support help primary care organizations succeed in value-based care. In 2024, 93% of Aledade ACOs earned shared savings — compared to 73% of non-Aledade ACOs. Aledade ACOs earned more than $775 million in shared savings payments, averaging approximately $390,000 per practice.

Doing Good for Society: Over the past decade, Aledade’s innovative care model has helped physicians deliver more preventive care that saves money for the overall U.S. health care system. Aledade directly supports physicians in under-resourced areas, with 61% of Aledade practices located in health professional shortage areas (HPSAs). Aledade has now generated over $3 billion in total health care savings. In 2024 alone, Aledade’s network cared for more than 1 million Medicare beneficiaries, generated over $1 billion in total Medicare savings, and saved taxpayers more than $320 million.

Doing Good for Employees: Aledade’s mission-driven workforce plays a vital role in its success, with over 90% of employees affirming the company’s commitment to its core value of service.
