HARTFORD, Conn. - June 22, 2026 - PRLog -- National Health Care Associates, Inc. (NHCA) is proud to announce that three of its Connecticut skilled nursing and rehabilitation centers have been selected to join UConn Health's Preferred Post-Acute Care Network, strengthening access to high-quality post-acute care for patients across the region.
The newly designated NHCA centers include:
- Maple View Health and Rehabilitation Center, Rocky Hill, CT
- The Hebrew Center for Health and Rehabilitation, West Hartford, CT
- The Pines at Bristol Center for Health and Rehabilitation, Bristol, CT

As members of the Preferred Post-Acute Care Network, the three NHCA centers will collaborate closely with UConn Health providers to facilitate smooth transitions from the hospital, support individualized care planning, and ensure continuity of care throughout the rehabilitation process.
