Partnership strengthens care transitions and expands access to high-quality skilled nursing and rehabilitation services for patients across Connecticut

HARTFORD, Conn. - June 22, 2026 - PRLog -- National Health Care Associates, Inc. (NHCA) is proud to announce that three of its Connecticut skilled nursing and rehabilitation centers have been selected to join UConn Health's Preferred Post-Acute Care Network, strengthening access to high-quality post-acute care for patients across the region.

The newly designated NHCA centers include: UConn Health's Preferred Post-Acute Care Network is comprised of post-acute care providers that demonstrate excellence in clinical outcomes, quality measures, patient satisfaction, and care coordination. The partnership is designed to support seamless transitions from the hospital to rehabilitation, improve communication between care teams, and enhance patient recovery.

As members of the Preferred Post-Acute Care Network, the three NHCA centers will collaborate closely with UConn Health providers to facilitate smooth transitions from the hospital, support individualized care planning, and ensure continuity of care throughout the rehabilitation process.
