On March 12, 2024, CCM Health notified individuals that the company experienced a data breach after an unauthorized party accessed sensitive consumer data entrusted to the company. According to CCM Health, they determined that information stored on their servers may have been subject to unauthorized access between April 3, 2023 to April 10, 2023. CCM Health determined that the information exposed in the data breach includes: full name, date of birth, Social Security Number, medical information (e.g., medical record number, patient account number, prescription information, healthcare provider’s name, medical diagnosis, diagnosis code, treatment type, treatment location, admission date, discharge date, and/or lab results), and health insurance information.
