On July 1, 2014, Actavis plc ("Actavis") completed its acquisition (the "Acquisition") of Forest Laboratories, Inc. This report reflects the beneficial ownership of the reporting person at the time of the consummation of the Acquisition and does not include the securities of Actavis acquired by the reporting person upon the consummation of the Acquisition. The reporting person will file a Form 4 reflecting her acquisition of Actavis securities in connection with the consummation of the Acquisition.  Exhibit List: Exhibit 24 - Power of Attorney
