(Attach extra sheets if needed.) As previously reported in a Form 8-K filed on March 23, 2001, CoBiz Inc. completed its acquisition of First Capital Bank of Arizona on March 8, 2001. The transaction was treated as a pooling of interests, and accordingly, all historical financial results are being restated to include the combined results of both entities.
